Position Type: PRN

Physical Therapist Responsibilities:
Perform evaluations to TIM patients/clients/residents
Provide appropriate treatment services to patients/clients/residents
Respond to evaluation/screen requests per department requirements
Establish treatment plans as appropriate per patient/client/resident.
Establish plan of care, administer, assist, demonstrate and supervise treatment measures.
Complete and submit written documentation on treatment services
Maintains confidentiality of all protected health information in any form.

Requirements:
Degree in Physical Therapy from an accredited program.
Completion of the National Certification Examination for Registered Physical Therapist
Physical Therapy license in the state of Ohio
