Therapy in Motion is actively seeking a PRN Home Health Speech Therapist to encompass evaluation and treatment or community based home health on the East Side of Cleveland. You would be working with a strong devoted team providing therapy services to patients within their homes. Benefits offered for this PRN position include 401k participation!

Duties/Responsibilities:

• Perform evaluations to TIM patients/clients/residents
• Provide appropriate treatment services to patients/clients/residents
• Respond to evaluation/screen requests per department requirements
• Establish treatment plans as appropriate per patient/client/resident.
• Follow established plan of care, administer, assist, demonstrate and supervise treatment measures.
• Complete and submit written documentation on treatment services
• Maintains confidentiality of all protected health information in any form.

Requirements:

• • Masters in Speech Language Pathology from an accredited program
• • Speech Language Pathology license in the state of Ohio
