Spiritual Care Coordinator
Embrace Hospice – Villa Rica, GA

The Embrace Hospice Spiritual Care Coordinator is responsible for the provision of spiritual care services to patients and families/caregivers of Embrace Hospice, either directly or through coordination of care with other spiritual counselors.

AREAS OF RESPONSIBILITY
A successful Spiritual Care Coordinator will be able to perform these essential duties and responsibilities. Reasonable accommodations may be made, in accordance with applicable law, to enable individuals with disabilities to perform the essential functions

  • Assure spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner. Provide direct spiritual care to patients and families/caregivers.
  • Facilitate the development in the interdisciplinary group (IDG) plan of care to meet identified spiritual needs.
  • Serve as liaison and support to community religious and spiritual counselors.
  • Maintain records of spiritual care services utilization and related activities for QAPI Program, program development, and policies and procedures review and revision.
  • Document direct services and ongoing communication with religious and spiritual counselors.
  • Attend patient care conference and interdisciplinary group (IDG) meetings as a member of the interdisciplinary group (IDG).
  • Provide consultation, education, and support to the interdisciplinary group (IDG) on spiritual care.
  • Recruit community religious leaders, spiritual counselors, and spiritual care volunteers adequate to meet patient and family/caregiver needs by developing community contacts and offering education through congregations about hospice care.
  • Provide for funeral and memorial services for patients as requested.
  • Provide bereavement follow-up services as requested and maintain proper records of visits to patients/families.
  • Plan periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team.
  • Assist in supervision of spiritual care volunteers when assigned to patients, families, and/or caregivers.
  • Expand and maintain community resource list as needed.
  • Assist in evaluating and planning services to meet hospice personnel support needs (i.e. yearly retreats, weekly support groups, etc.)
  • Function independently, but seek and accept guidance from other members of the interdisciplinary group, or from member(s) of the community.
  • Participate in hospice activities such as in-service education, hospice personnel meetings, and relevant communities.
  • Assist in orientation of new employees.
  • Market and provide community education.
  • Participation and marketing in public relations and community activities that promote the organization and its role as an effective member of the hospice and healthcare delivery system as well as customer service promotion.
  • Must be available during normal work hours (unless previously approved by direct supervisor). Additional hours may be required to complete normal business functions and/or projects.
  • Perform other duties as requested or required, in the sole discretion of Embrace.

  • Demonstrates or verbalizes knowledge and understanding of Medicare CoP’s, licensing, and state standards for hospice volunteer coordination.
  • Able to verbalize hospice goals and philosophy.

Necessary skills, attributes and requirements

  • Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree).
  • One (1) Clinical Pastoral Education (CPE) unit is preferred.
  • Comfort in an interfaith setting.
  • Ability to accept differing lifestyles, cultures, beliefs, and values.
  • Ability to network with community clergy and congregations.
  • Previous experience in hospice and/or related healthcare setting is preferred.
  • Documented evidence of experience and/or education in grief or loss counseling.
  • Ability to read and communicate effectively. Strong written and verbal skills. Should demonstrate good interpersonal and communication skills under all conditions and circumstances.
  • Basic computer knowledge.
  • Ability to foster a cooperative work environment.
  • Team player with ability to manage multiple responsibilities and demonstrate sound judgment.
  • Must be able to work flexible hours and travel between offices, facilities, etc.
  • Understand hospice philosophy and issues of death and dying.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organizational requirements and is in good working order.
