Always Best Care in Middleburg Heights, OH is now hiring a full time Clinical Coordinator Nurse!
M-F: On call every other month for one weekend.

Requirements:
- Self-starter who is passionate about patient care
- Knowledgeable about Medicare rules and regulations
- Can coordinate proper reporting
- Reviewing documentation submitted by clinical staff and coordinating care with the RN/Case Manager
- Must be able to communicate well with staff in regard to corrections, patient's status and completeness of documentation.
- Must be able to work well in an office setting
Qualifications:
- Must have LPN License
- Must have at least 3 years experience
- Home care background is preferred
- Computer skills is a must
