Extended Housing Inc. is currently accepting applications for a full-time Housing Support Worker to assist severely mentally disabled and/or emotionally disturbed individuals and families in obtaining and maintaining permanent housing. Schedule: 8:30a-4:30, must be flexible as needed.

Essential Duties:
Assist in completing and submitting applications, renewal applications and annual progress reports for Shelter Plus Care program as needed.
Participate in the Inter Agency meeting as scheduled by Director of Housing Services (as needed).
Inform treatment team of warnings given and positive progress of participants on program.
Enter and update all Shelter Plus Care participants in database accurately.
Maintain all participant records.
Accept and organize documentation from program participants for required Participant Match.
Prepare for and participate in all required audits and reporting.
Assist participants in identifying housing needs and problems. Address these as able, including making referrals and acting as an advocate.

Qualifications:
Must experience in working with persons experiencing mental illness
Must have 2 year degree in social work or related field preferred
Must have working knowledge of computer and Microsoft Office programs
Ability to learn and maintain knowledge in tenant/landlord law, Fair Housing, and the ADA
Must be comfortable making home visits throughout the community and transporting participants in personal vehicle
Must be able to lift 20 lbs
